The following steps need to be taken in order to reserve your spot:

1. Register Online by Monday, June 1, with a $200 Deposit - Click here to register!

*Note* When it comes to picking either gold, silver, or bronze skill level divisions, please email us at [email protected] to find out what your team's regional qualifier ranking is before filling that part out on the online registration form.

2. Complete Roster & Host Hotel Information Sheet - Fax to (312) 578-3882 by Thursday, June 16.

The following will be accepted before your first game at the National Tournament:

• RHA Membership is required - #'s or forms must be submitted with registration
• Completed Individual Waiver Form
• Proof of Age (Youth Only)
• Remaining Entry Fee of:

$525 –Youth, 21u , Adult Gold/Silver/Bronze, & 35+
$795 – Adult Elite Teams Only
