* All games will take place at:
East Valley YMCA's Dave Potell Memorial Hockey Rink
11455 Magnolia Blvd., North Hollywood, CA 91601

Team Sign Ups:
Fee: $880 per team
* Once your TEAM is confirmed, a TEAM NUMBER will be assigned.
* This TEAM NUMBER must be presented at time of payment.
*Payment must be made in full by the week of July 16, 2012.
*TEAM CAPTAINS must make complete payment in full before team play.
* Partial payments are not accepted.

* We will be sure to add you to the Free Agents List and help in finding a team for you.
