I am conducting a study on head injuries in hockey. I would like to cite a firsthand resource, such as yourself, as a reference. If you could take a minute to answer the following questions, I would greatly appreciate it. Any responses can be returned to my e-mail address or posted. Thank you very much.
This is intended for coaches, team trainers, or doctors.

What is your name, title, and the nature of your work?

How long have you been around hockey (experience, etc.)?

What kinds of injuries to the head do you treat most often?

Could these be prevented and if so, how?

What head gear do you recommend for hockey players?

Have you noticed any significant advances in hockey protective equipment technology recently and if so, are any notable?

What are some consequences for not wearing proper equipment (helmets, etc.)?

What is the worst head injury you have treated/seen?

Do you think helmet standards today are strict enough?

How would you improve a players safety (especially to the head) while playing hockey?

Please list any other comments/advice that could be helpful:

THANK YOU VERY MUCH
